  • Board Certification or Eligibility Proof
  • Updated CV in Month / Year format (i.e. June,2020 - June 2024). No unexplained gaps longer than 30 days.
  • Professional References – List supervisors/colleagues who respond quickly
  • Malpractice Insurance & Claims History
  • Immunization & Health Records – TB, Hep B, MMR, Varicella, Tdap, Flu, COVID-19 (if required)
  • Case Logs (if applicable) – Last 12-24 months for procedural privileges
  • Hospital Affiliations (if applicable)
  • Perform NPDB Self Query
